We are still welcoming registrations from new patients during the Covid-19 outbreak. Please contact the practice for more information or complete the online registration form below.
If you'd prefer to call in to the surgery to submit the forms, you can download them here:
GMS1 Registration Form
New Patient Questionnaire
We can also email you the forms, you can contact us via telephone or through our online contact form on this website. You are required to complete a registration form for each member of the family and a brief questionnaire as your records could take a while to come over from your previous GP.
New Patient Health Checks
All new patients are requested to complete a health questionnaire. We recommend that patients with a chronic disease come for a health check-up.
East Parade is in a fully accessible building with disabled parking bays. There are disabled toilets with baby change facilities.
In the interests of patient and staff safety we have CCTV in the building and car parks.
Where to Find Us
We are based at the Mowbray Square Medical Centre, Harrogate, HG1 5AR. Click below to see where we are on the map.
East Parade Surgery Map
The Surgery is in a central Harrogate location with easy access by bus (111,104, 100, 101, 102), train, taxi, private car (we have limited parking) and on foot.
Al practices have an Inner and Outer practice boundary. We are not able to have patients registered on our list who live outside the outer boundary area.
- The practice 'inner boundary' is the area from which new patients can register and covers patients living in Harrogate, Killinghall, most of Beckwithshaw, Pannal, Spacey Houses, Bilton, Forest Lane Head and Knox.
- The practice 'outer boundary' includes our inner boundary plus the area existing patients may move to and still remain on our practice list i.e. Killinghall.
- If you are unsure whether you are eligible to be registered at the practice, please enter your postcode in the box next to the map below.
If you move out of the Harrogate area, you will need to find a new GP practice.
